Our home-based services are a family-focused therapy service performed in the home and community for children and adolescents with severe emotional and behavioral health disorders. Our service aims to reduce the likelihood of out-of-home placement for clients who may be at risk of hospitalization or other community placements because of their mental health, as well as those transitioning to homes from higher levels of care.
The home-based team meets with the family a minimum of two times each week and provides the following services:
The therapist works closely with all involved systems (case workers, schools, health providers, courts, etc.) to provide appropriate support and assistance to the family. Home-based therapy is provided as a supplement to outpatient therapy when it is determined that additional services are necessary.
Referrals for home-based services should be made through your primary therapist whenever possible. They will relay information to the home-based team who will then set up an assessment to discuss the components of home-based services and to ensure that the service is appropriate for your family.
